If you would like us to send you our Joining Parkway Dentalcare pack then either:
Telephone 023 9247 5086, and speak to any one of our friendly reception team. If they are busy dealing with patients please leave your contact details including your address and telephone number.
Mail It is possible for us to send out new patient forms to patients who might find it hard to visit the surgery. Give us a call or fill in our contact form to request the new patient pack.
Visit If you are live close to our surgery, then feel free to drop in and pick up your new patient forms from our receptionist who will be happy to help you.
Complete the forms and return them to us. We need these forms before we can book you an appointment.
To enable us to give you the best and safest treatment we need to be aware of all aspects of your health, therefore we require all new patients to fill in a Medical History Form, which your dentist will discuss fully before commencing any treatment.
Alternatively you can complete your medical history form when you arrive for your initial appointment where our receptionist will provide you with any help you need to complete it accurately.
IMPORTANT please turn up 15 minutes prior to your appointment if you wish to complete your medical history form before your initial appointment.
Parkway Dentalcare has undergone some changes recently. We now provide different types of care to give our patients the best oral health care available. Please read your new patient pack and asses what provision of care you need.
Please fill out your smile assessment form, which has been included to help us understand your oral health needs so we can provide you with the best care possible.
Only once we have received your completed New Patient forms, we will be able to book you an appointment.
We have appointments available most days at times convenient to our patients. If you call us on 023 9247 5086, we will endeavour to book you the next available appointment.
